Obituary

Theresa E. "Mom" (Zelasko) Kozak, 86, of Dunbar, PA, passed away peacefully on Thursday, October 15, 2015, at home, with her loving family by her side. She was born, March 20, 1929, in Melcroft, PA, a daughter of the late Frank and Mary (Serbo) Zelasko.

In addition to her parents, Theresa was also preceded in death by her husband, Emil J. Kozak; a son, Lawrence M. Kozak; an infant daughter, Renee Kozak; and her loving companion, Jake Miller; sisters, Sophia Faust, Florence Martinowski, Bridgett Sanders; and brother, Frank Zelasko Jr.

Theresa was a member of St. John the Evangelist Roman Catholic Church, in Connellsville, PA. She retired from Connellsville Area School District where she worked as a cook and a baker. She was the owner and operator of the Kozak Catering Company for over 30 years and was very proud of her home cooked Polish Food. Theresa volunteered to cook for many different organizations to raise money for fund raisers. Her cooking and willingness to help others culminated in receiving the West Leisenring VFD 2002 Firefighter of the year award. She was very proud to be the mother of a Soldier, a Sailor, and a Marine, and was proud to be an auxiliary officer, Conductress, in the North Union VFW post 8543, she was also a member of the Valley Sportsmans Club.

Left to cherish her memory, are her children, Ronald J. "Crow" Kozak and Barb, Christine A. Tinker and Alex, Emil J. "Buddy" Kozak Jr. and Judy, Susan M. Griffith and Buck, and Connie White, whom she called her own, and Doug; grandchildren, Michael Kozak and Jillynda, Jocie Kozak, Amanda Hixon and Doug, Emiley Donaldson and Jason, Alex Tinker IV, Hillary Griffith, Alexandra Griffith, Megan Johnson and Steve, Brittany Sapp and Bryan, and Craig Konopa; great grandchildren, Kelsie, Amelia, Craig "CJ", Brendan, Sara, Noah and Preston, Nya, Hunter, Bristol, Owen, Charleigh Ann and Bryson; sisters, Catherine Pritts and Bob, Stella Free, Mary Louise Mullen and Jack; and many nieces and nephews.

Friends will be received at the Stephen R. Haky Funeral Home, Inc., 603 North Gallatin Ave. Ext, Uniontown, PA., on Monday, October 19, from 2-4 and 6-8 p.m. and Tuesday, October 20, until 11 a.m. when a Blessing Service will be held in the Funeral Home Chapel, with Rev. Father Bob Lubic Officiating.

Interment will be privately held at St. Vincent De Paul Cemetery, Leisenring, PA.
