Lydia A. Forman, 85 of Uniontown, Pa., passed away at home on Sunday July 17, 2016.
She was born February 26, 1931 in McClellandtown, Pa., a daughter of Thomas and Zeda (McCloy) Hostetler.
Left to cherish her memory is her husband Vernon J. Forman, children Alan D. Forman and his wife Judy, David K. Forman, Todd E. Forman, Tracy R. Lange, grandchildren Jonathon, Evan, Dylan, Connor, Madison and Lexi.
Arrangements were private and under the direction of The Stephen R. Haky Funeral Home, Inc. 603 North Gallatin Ave. Ext., Uniontown, Pa.
Entombment was privately held in Sylvan Heights Cemetery, Chapel of Memories Mausoleum, Uniontown, PA
